The form collects the information needed to identify the responsible adult, provide details about the child, make an election for a Pilot Program Contribution when applicable, and provide consent for the IRS, Treasury, and their authorized agents to create and maintain the account.

The form is designed for individuals who have a child eligible for a Trump Account and want to request that an account be established for that child. For each eligible child for whom an account is requested, the responsible individual must complete the required sections of the form. The form includes information about the adult making the request, the child or children included in the election, eligibility for the $1,000 Pilot Program Contribution, and authorization for information disclosure related to account creation.

Part I collects information about the adult who is requesting the Trump Account. The individual listed in this section becomes the responsible party for the account.

First Name

Enter the legal first name of the parent, guardian, or authorized individual completing the form.

Middle Name

Enter the individual’s middle name if applicable.

Last Name

Enter the individual’s legal last name.

Social Security Number

Provide the Social Security number of the parent, guardian, or authorized individual responsible for the account.

Home Address

Enter the complete residential address of the responsible individual. Include the street number and street name.

If the individual uses a P.O. box, additional instructions may apply.

Apartment Number

Enter the apartment, unit, or suite number if the home address includes one.

City, Town, Or Post Office

Enter the city, town, or post office location associated with the address.

County

Provide the county where the address is located.

State

Enter the state for the home address.

ZIP Code

Provide the ZIP code for the address.

Foreign Country Name

If the responsible individual has a foreign address, enter the name of the foreign country.

Foreign Province/State/County

For a foreign address, provide the applicable province, state, or county information.

Foreign Postal Code

Enter the postal code used for the foreign address.

Date Of Birth

Provide the birth date of the parent, guardian, or authorized individual.

Phone Number

Enter a current phone number where the responsible individual can be contacted.

Email Address

Provide an active email address for communication purposes.

Part II: Child’s Information

Part II collects information about the eligible child or children for whom the Trump Account is being requested. The form provides sections for Child 1 and Child 2. If more than two children are involved, additional instructions should be reviewed.

Complete the information separately for each child included on the form.

Enter the child’s legal first name.

Enter the child’s middle name if applicable.

Provide the child’s home address information.

If the child’s address is the same as the address listed in Part I, check the box indicating that the address is the same. Otherwise, complete the separate address fields.

Enter the child’s street address.

Line 5b: Apartment Number

Provide the apartment or unit number if applicable.

Line 5c: City, Town, Or Post Office

Enter the child’s city, town, or post office location.

Line 5d: County

Enter the county associated with the child’s address.

Provide the state where the child lives.

Line 6: Authorized Individual Checkbox

Check the box if the person completing the form is authorized to open the Trump Account for the child. Additional instructions may apply regarding authorization requirements.

Part III: Pilot Program Contribution Election

Part III allows the individual completing the form to request that an eligible child receive a $1,000 Pilot Program Contribution to their Trump Account.

A child must meet the listed requirements to qualify. The form states that the child must have been born during 2025 through 2028, must be a qualifying child of the individual opening the account, must be a U.S. citizen, and must have a valid Social Security number.

Line 7: Pilot Program Contribution Election

Check the box if the child qualifies for the Pilot Program Contribution and the responsible individual wants the child to receive the contribution.

This election must be completed separately for each eligible child listed in the form.

Part IV: Consent To Disclose Information

Part IV explains the authorization provided by completing Form 4547. By submitting the form, the individual authorizes the IRS, Treasury, and their agents to create and maintain a Trump Account for the eligible child or children listed on the form.

The consent also allows the IRS, Treasury, and their agents to disclose that a Trump Account has been established to an eligible parent, guardian, or authorized individual who is permitted to request account creation.

Enter the date the form is signed.

The declaration states that the signer confirms, under penalties of perjury, that they examined the form and believe the information provided is true, correct, and complete.

This section is completed only by a paid preparer if someone other than the taxpayer prepares the form.
